Transition Services

The intent of transition services is to aid individuals, who wish to relocate from nursing facilities back into the community. Staff helps to coordinate the numerous tasks that must be accomplished prior to such a move, including establishing an income, locating housing, acquiring furniture and household items, recruiting an attendant, setting up utility services, and assuring that the consumer has access to transportation and medical care. This service is extremely time intensive and usually several months are required to facilitate relocation back into the community. At year’s end, five clients were engaged in this process.
